Scheduling Appointments

New patient consultations and follow-up visits are scheduled through our office staff. Please have your insurance information and relevant medical records available when requesting an appointment. Referring physicians are welcome to contact our office directly for expedited consultations

Elective Surgery Scheduling

When elective surgery is scheduled, operating room time, anesthesia services, and clinical staff must be coordinated in advance.
To reserve operating room time, a $100 scheduling deposit is required. The deposit is applied to the patient’s surgical account and is not an additional fee.

Cancellation or Rescheduling

More than 7 days before surgery: the deposit will be refunded or applied to the new date
Within 7 days of surgery: the deposit may be forfeited
Reasonable exceptions may be made for medical illness, emergencies, or other unavoidable circumstances.

Insurance and Financial Responsibility

Patients remain responsible for copayments, deductibles, coinsurance, and any non-covered services as determined by their insurance plan.
Our office staff will assist with insurance verification and provide estimates of expected patient responsibility when possible.
